Key features
- Two levels of memorymain memory file (automatic injection, truncation within budget) + topic file (progressive disclosure: only one row of index is injected, and the text is loaded on demand)

Requirements
- Concurrent writing of the same item across processes is a weak guarantee of "the last write wins" (strict serialization within the process)
- Memories are shared by project, not segregated by session: any session on the same project can overwrite or delete entries recorded by other sessions.
