主要功能
- 每个挂载条目必须有 id。
- 链头就是请求本身（用户在 harness 首页聊天栏选的 provider/model，或部署默认值）。
- llm/fallback——每次切换时追加。
- 新增条目必须放在顶层 - insert列表里（参照 harness 的 examples/web-schedule/cordis.yml）。

使用条件
- 单一全局备用列表所有请求共享一个 fallbacks 列表
- 失败按实际服务的 (provider, model) 归因，一个 agent 的成功不会清除另一个 agent 的待定计数。
